Michael Yaremchuk, M.D.: A New Paradigm for Breast Cancer Treatment and Reconstruction

When a celebrity is diagnosed with a serious illness, considerable attention is drawn to that disease. Most recently, E! host Giuliana Rancic's decision to have a double mastectomy after being diagnosed with breast cancer has drawn national attention to an evolving paradigm for the treatment and rehabilitation of this disease.

Breast cancer affects around one in eight women during their lifetime in the USA. It is the second largest cause of cancer mortality. This year, almost 300,000 U.S. women will have been diagnosed with some form of breast cancer. A new trend in the treatment and rehabilitation with women diagnosed with breast cancer has evolved in many major medical centers.

When Dr. Eric Liao, a plastic surgeon at the Massachusetts General Hospital, and his colleagues reviewed the surgical treatment of over 3,000 patients during a 10-year period. He compared treatment and reconstructive modalities of women treated in the period between 1999 and 2004 with those treated in the period between 2005 and 2010. He found that in the more recent group, younger women were being diagnosed and treated for breast cancer, and that more women chose to have bilateral mastectomy (similar to Ms. Rancic) and reconstruction with implants.[1]

Over the last decade, the mean age at the time of diagnosis of breast cancer has decreased. In Dr. Liao's study, it decreased by seven years. This is a result of increased patient awareness, improved mammographic techniques and genetic testing. Discovery of a breast cancer at an early age increases the likelihood of cure, decreases the morbidity of treatment and broadens the spectrum of reconstructive options.

Dr. Liao's group found that there was more than a 2.5-fold increase in the number of women choosing bilateral mastectomy in the more recent patient group,. The majority of these underwent reconstruction with implants. Several factors impact on this decision. Patients found to have the BRCA gene mutation, which predisposes one to develop breast cancer have a high likelihood of developing breast cancer in both breasts. Hence, they choose to have both breasts removed at the time of breast cancer diagnosis. Removing both breasts predisposed to cancer avoids the anxiety of monitoring for the appearance of a cancer in the opposite breast.

An improved aesthetic outcome is particularly appealing to younger patients who are undergoing mastectomy. Reconstructing a breast after unilateral mastectomy to match the opposite un-operated breast is extremely challenging and often requires the surgeon to operate on the uninvolved breast. Bilateral mastectomy with immediate implant reconstruction avoids this problem thereby optimizing the chances for symmetry and improved aesthetic outcome. It is important to note that breast implants do not impair breast health. Careful review of scientific research conducted by independent groups such as the Institute of Medicine has found no proven link between breast implants and autoimmune or other systemic diseases.

Even with ongoing procedural advances, it's important to keep in mind that a reconstructed breast will not have the same sensation and feel as the breast it replaces, and visible incision lines will always be present on the breast, either from reconstruction or mastectomy. But overall, reconstructive surgery can give women new breasts that feel natural and have an aesthetically pleasing appearance.

Detroit Man's Shocking HIV/AIDS Discrimination Claims Pick Up Steam

More than 35,000 people have called for the resignation of the regional director of Detroit's Great Expressions Dental Care after a former employee claimed he was subjected to horrific levels of HIV-related discrimination while on the job.

According to the Change.org petition, James White was "banned from touching doorknobs, was followed around by coworkers with bottles of Lysol, and subjected to sudden and abrupt schedule changes" after being diagnosed with HIV, even though he did not work directly with patients in his position as a billing clerk. While he endured the on-the-job discrimination for seven months, the 26-year-old White was terminated from his position as a billing clerk for "excessive, unexcused absences" after being hospitalized for a week.

"I felt like my character was destroyed,” White told POZ earlier this month. "I went from wanting to be an activist -- someone who spoke to groups about HIV -- to someone who didn’t want to leave my room for six months."

The petition, which currently has about 37,470 signatures, was started by Jim Harris, a University of Oklahoma student who also directs an HIV/AIDS group on his campus. "It's absolutely tragic and shocking that thirty years after HIV was first discovered, we're still seeing vile acts of discrimination toward those who are HIV-positive," Harris is quoted by The Ionia Sentinel-Standard as saying. "Nobody should have to experience the type of discrimination and harassment that James White suffered. This is too atrocious a crime not to evoke public outrage."

According to MLive, White has received a “right to sue” letter following a lengthy Equal Employment Opportunity Commission investigation, and his lawyers are in the process of filing an anti-discrimination lawsuit. A Great Expressions Dental Center spokesman has previously said the company does not discuss current or former employees and declined further comment on the case.

HIV/AIDS advocates have condemned Great Expressions, a national chain which operates 150 offices in Connecticut, Florida, Georgia, Massachusetts, Michigan, Ohio, and Virginia. "HIV is not spread by casual contact or touching surfaces other people may touch. We're glad that the former employee is standing up for his rights," Peter Kronenberg, vice president for communications for the National Association of People with AIDS, told POZ. "[If the allegations prove to be true], he sends a message to other employers and HIV-positive employees everywhere that workplace discrimination is unacceptable."

But a few minutes into it, she decided it was too hard, and she really wasn't dressed appropriately for exercise anyway. "I have some friends here that might show you what to do. Okay?"

With those words, a flash mob erupted on the set, with people running in from all around, and even large portions of the audience getting up and getting involved in the choreographed exercise routine. It was certainly a hip and modern way to bring a sense of fun to fitness, proving that Jane Fonda is just as current today as she was decades ago.

She's keeping busy with more than just exercise as well. Fonda has signed on for a recurring role on Aaron Sorkin's untitled HBO drama series set at a cable news network. Reports have Fonda playing the CEO of the network.

If you think it's OK to eat unhealthily while waiting for your flight at the airport, a new report shows you might be out of excuses.

Eighty-three percent of airport restaurants have at least one heart-healthy, low-fat, cholesterol-free vegetarian entree on their menus, according to an analysis of 15 of the country's busiest airports. The analysis, which has yet to be pub online, was conducted by the nonprofit Physicians Committee for Responsible Medicine.

"Healthy airport food has taken off over the past decade, but there is still room for improvement," PCRM nutrition education director Susan Levin, who is a registered dietitian, said in a statement. "The good news is that healthy vegetarian choices are available now at most airports. But some terminals are still clogged with high-fat, high-cholesterol offerings like pizza and cheeseburgers."

PCRM began doing the rankings in 2001, when just 57 percent of the reviewed eateries offered a healthy option.
